Concurrent Pacemaker Lead Perforation and Subacute Coronary Stent Thrombosis: A Case Report

چکیده
Stent thrombosis and lead perforation are important differentials for patients presenting with chest pain following recent coronary stent insertion pacemaker insertion. In this report, we describe an unusual case of a 78-year-old male who presented sharp one week after admission posterior ST-elevation myocardial infarction (STEMI) subsequent Mobitz type II block, which he received primary percutaneous intervention (PPCI) to the left circumflex artery (LCx) dual chamber permanent (PPM) Computed tomography (CT) CT angiogram (CTCA), respectively, showed had concurrent thrombosis. On balance, cause was likely perforation. This diagnosis reached by having high index suspicion both these post-procedure complications investigating appropriately.

The potential hazard of drug-eluting stent-induced coronary vasospasm causing subacute stent thrombosis: a case report
BACKGROUND Drug-eluting stent (DES) -induced coronary vasospasm is a well known phenomenon after stent implantation; however, the extent of this risk is still unknown. We report a case in which DES-induced severe coronary vasospasm was clinically suspected as a cause of subacute stent thrombosis (ST). CASE PRESENTATION A 67-year-old man came to our hospital due to chest pain with mild exercis...
متن کاملAcute and Subacute Stent Thrombosis in a Patient With Clopidogrel Resistance: A Case Report
Drug-eluting stents (DES) are considered the treatment of choice for most patients with obstructive coronary artery disease when percutaneous intervention (PCI) is feasible. However, stent thrombosis seems to occur more frequently with DES and occasionally is associated with resistance to anti-platelet drugs. We have experienced a case of recurrent stent thrombosis in a patient with clopidogrel...
متن کاملFibrin glue patch for pacemaker lead perforation of the right ventricular free wall: A case report
Introduction Pacemaker lead perforation is a complication associated with screwing in the pacemaker lead. Nonsurgical management of this complication is limited. Pericardial drainage is performed for the resultant cardiac tamponade, but it is difficult to stop bleeding from the perforation site without surgery. Here, we describe a case in which fibrin glue injected via a 6 F pericardial tube in...
متن کاملCase report: Pacemaker lead perforation of a papillary muscle inducing severe tricuspid regurgitation
INTRODUCTION We report a rare but severe pacemaker complication of a pacemaker lead perforating the papillary muscle. This induced severe tricuspid regurgitation and right heart failure. Patients suffering from right heart failure have an increased operative risk of open-heart surgery and therefore represent a clinical challenge due to the lack of clear guidelines. CASE PRESENTATION A 70-year...
متن کاملSubacute Right Ventricle Perforation by Pacemaker Lead Presenting with Left Hemothorax and Shock
Cardiac perforation by pacemaker is a rare but potentially fatal complication. Acute perforations occurring within twenty-four hours of insertion of pacemaker can lead to hemopericardium, cardiac tamponade, and death. Hemothorax occurring as an acute complication of pacemaker insertion is reported but extremely rare. Previously, hemothorax and shock as a subacute complication following pacemake...
